2012 Summer Camps
Registration Instructions

If you would like to register for one of our summer camps please do the following:

1. Print and complete the Summer Camp Registration Form.
2. Be sure to check the camp(s) you are registering for.
3. Print and complete the Waiver Release Form (be sure to sign form).
4. Return the completed forms along with your check for the total amount for the camp/camps you sign up for payable  to The Sports Shack.

   If you would like to pay by credit card (Visa or Mastercard), please download the credit authorization form. Complete the form and mail it with your sign up form and signed waiver release.

5. Once we receive the Two Forms and Payment we will confirm by email that your  registration is complete.
